PTF Enrollment Form

We would like for you to be involved as a member of PTF. If you have questions, please do not hesitate to contact an officer listed below.  Meetings are scheduled “as needed” and anyone is welcome to attend.  E-mail notification of meetings is sent to parents.  If you are unable to attend meetings, your “volunteer” help is greatly appreciated.  It takes the support of many to make PTF activities a success.  Please complete the information below and return it to the TMP Office.
